Objective evidence is verifiable information based on fact: records, measurements, test results, signatures, certificates, and similar artifacts that demonstrate a required activity was performed and a requirement was met. The word that matters is objective: the evidence must be independently verifiable, not dependent on the recollection or assurance of the person who did the work. Every process in a nuclear quality assurance program either produces objective evidence or depends on it. It is what distinguishes quality that can be demonstrated from quality that is merely claimed.
What counts as objective evidence
Objective evidence takes many forms, but each shares the property of being verifiable after the fact by someone who was not present when the work was done. Common examples in nuclear work include inspection and test reports, calibration certificates traceable to national standards, material test reports and certificates of conformance, qualified procedure records, signed and dated work packages, radiographs and other examination results, training and qualification records, and audit reports.
What these have in common is that they capture a fact, not an opinion, and they identify the specific item, the specific requirement, the method used, and the person responsible. A measurement recorded with the instrument used and its calibration status is objective evidence. A note that says "checked, looks good" is not: it cannot be verified, and it does not tie back to a requirement. The rigor of nuclear documentation exists precisely so that records qualify as objective evidence rather than as after-the-fact assurances.
Why it is the foundation of nuclear QA
The nuclear industry cannot rely on trust or memory to establish that safety-related work was done correctly. Components are often inaccessible once installed, defects can be undetectable after the fact, and the people who performed the work may have retired long before a question about it arises. The only durable answer is a record that stands on its own. This is the practical meaning of the industry maxim that if it is not documented, it did not happen: an activity for which no objective evidence exists cannot be shown to have met its requirements, and so, for regulatory and safety purposes, it is treated as if it did not occur.
Objective evidence is why nuclear QA is document-heavy, and why that is not bureaucracy. Each record is the proof that a specific requirement was met by a specific qualified person using a specific approved method. Remove the record and you remove the ability to demonstrate safety, no matter how well the work was actually performed.
Objective evidence is required throughout the standards. It is what an audit or assessment examines, what a non-conformance report and corrective action generate as they document a problem and its resolution, and what inspection and test control produces at each verification point. Every quality process in this glossary ultimately exists to create, control, or rely on objective evidence.
Creating evidence versus preserving it
Objective evidence is only useful if it can be retrieved when needed, which may be decades after it was created. Generating good evidence and preserving it are two distinct disciplines. Records management is the QA process, required under NQA-1 Requirement 17, that governs how quality records are identified, stored, protected, and retained for the life of the plant and beyond. Evidence that was created correctly but cannot be located, or that degraded because it was not properly preserved, fails the same test as evidence that was never created.
This retrieval requirement is where many quality programs struggle in practice. When objective evidence is scattered across document repositories, spreadsheets, email attachments, and paper files, producing the complete evidence trail for a single item on demand becomes a manual investigation. When the requirement and its supporting evidence are held together in one controlled system, the evidence trail is available immediately, and audit readiness stops being a periodic scramble.
